The Rule:

Note:
I always thought it was when "the ball reaches the PLATE". We're splitting hairs here but if the batter was at the front of the box the runner could leave before the ball reaches the plate.

Note:
The "star" indicates a part of the rule I don't think people are aware of. If a runner leaves early in any of the situations indicated. No run scores.

Note:
The "star" describes the logic behind this rule.
Note the last part of the instructor comments. To summarize further, in this example if the ball was bunted and a runner left early, the runner who WAS on third will be removed since there is no empty bag to put him on. No OUT is recorded.

Important things to note:
If the runner leaves early and there is no play, this is a warning sign that there is a good chance he will do it when there is a play.
Easiest call is when the play allows you to return the runners to their original bases.
We don't penalize the batter for his teammates' mistake.
Softball
MUCH simpler. I'll address that at a later time.
Examples
There are several examples in the Little League Rules and the RIM. I recommend you read this section.
